Integrated Service Framework
Trauma Recovery & Wellness Care is Healing Hearts’ trauma-informed, whole-person service framework designed to help Veterans, caregivers, trauma survivors, families, and community members identify support needs and access appropriate care and resources.
Through screening support, individualized care navigation, caregiver support, resource coordination, referrals, and collaborative community partnerships, the framework helps reduce barriers to care, strengthen continuity of support, and connect individuals with qualified healthcare and behavioral-health providers and other appropriate community services.
Healing Hearts does not present this framework as a separate clinical treatment service. Diagnosis and clinical treatment are provided by appropriately licensed or otherwise qualified healthcare and behavioral-health professionals.

Developing relationships with qualified professionals, healthcare and behavioral health providers, licensed and ordained ministers, ministries, Veteran-serving organizations, and community resources to support consent-based referrals and reduce barriers to care.
